End Of Life Planning

Dying with Dignity Tasmania supports the right of everyone to express their end-of-life wishes and to have them respected.

We encourage everyone to undertake advance care and end-of-life planning, and to ensure that those close to you know what you would prefer in different health and life circumstances.

We recommend appointing an Enduring Guardian, who can speak for you and carry out your wishes, if you are unable to do so. Forms may be obtained from Service Tasmania, or the Tasmanian Civil and Administration Tribunal by phoning 1800 657 500, or from their website.  We also recommend completing written instructions in the form of an Advance Care Directive.

End-of-life planning guide

DWDTas has for many years had a major focus on encouraging, supporting and helping people to do their end-of-life planning, express their wishes through advance care directives and where possible to appoint an Enduring Guardian.  Bill Godfrey, who for many years was the DWDTas Vice-President, did so much of this work and also developed and updated our much-requested End-of-life Planning Guide. There are many of us who are very grateful to Bill for this.

Bill has now worked with the Council on the Ageing (COTA) to make the Guide more accessible in a very attractive publication which will continue to be maintained by COTA.  COTA is also offering information sessions.  The Guide can be found at  https://www.cotatas.org.au/programs/end-of-life-planning/ or contact COTA on (03) 6231 3265 or by email at admin@cotatas.org.au.

Apart from practical advice on document storage, your will and funeral arrangements, the Guide provides links to relevant forms and covers the following areas:

  1. Enduring Guardianship
  2. Advance Care Directive
  3. Medical Goals of Care
  4. Organ Donor.
